Welcome to the SALPN Online Complaint Form

The SALPN takes all complaints very seriously. Please be sure to provide as much detailed information as possible to help facilitate the investigation process. This form will take approximately 10-15 minutes to complete, depending on the number of details you need to provide.

To submit your complaint you will need to have the following information available:

  • Your complete contact information
  • The LPN's full name and workplace
  • Specifics of the incident that led to your complaint, including location, date/time and supporting documents if available.
  • Any witness contact information

You will be required to agree to the following terms before submitting your complaint:

  1. The LPN in question will be provided an opportunity to respond to the concerns outlined in the complaint.
  2. The SALPN will provide the LPN with a copy of the complaint as received and may share some or all of the information and documentation collected during the investigative process.
  3. As the complainant, your name will be released to the LPN in question, the SALPN Investigation, Counselling & Investigation Committee and the Discipline Committee for the purpose of conducting a thorough investigation.
  4. As the complainant, I will allow for a recorded interview with a SALPN complaints investigator, if requested.
